MEMBERSHIP APPLICATION/SIGN-UP FORM
Your eBook should ALWAYS have a membership application or sign-up form inside, usually in both the early part of the book and at the end. Your reader may have received a free ebook from a friend and was not the original recipient. Your website should have a required sign-up form for the original recipient to fill out BEFORE they can download the product. Of course, if you're selling the book instead of giving it away, then you practically have to have the sign-up information ... how else could you send them the product?
